The pedigree must be in your name, so when consulting the RG/STUD BOOK on the CONSULT PEDIGREE button on the breeder panel, click on the icon              REQUEST DNA, the form will appear.

Just click the button and follow the instructions.  This procedure is automated directly with the laboratory, the exam response will also be electronic. ​
